The direct trocar entry technique, without preinsufflation, requires elevation of the anterior abdominal wall with the nondominant hand while inserting the primary sharp trocar directly and blindly towards the peritoneal cavity. Once the tip of the trocar has been inserted through the skin incision, the trocar is pushed through the fascia and the muscle layer by a continuous twisting motion with constant downward pressure, so that the surgeon can easily realize when the trocar has pierced the peritoneum and entered the peritoneal cavity. The Jain point is located in the left paraumbilical region at the level of umbilicus, on a straight line drawn vertically upwards from a point 2.5 cm medial to ASIS. The 5 mm trocar is inserted at this point but 10 mm trocar can be used either for direct trocar insertion, depending on your port configuration and you can keep Jain point trocar through your surgery, as well. Then telescope inserted into the abdomen after then pneumoperitoneum created from Jain point trocar and whole abdomen inspected. Other secondary ports are inserted under vision of this telescope. If adhesions are present, adhesiolysis is performed and then according to your port configuration ports are placed.
