The needle is kept exactly in the midline, is first inserted through the sacrococcygeal membrane making an angle of 45° to the skin. A distinct “pop” or “click” is felt as the sacrococcygeal membrane is punctured. Once through the membrane the needle is depressed a further so that the needle shaft is almost parallel to the skin. The bevel should face anteriorly and advanced 2–3 mm to enter the caudal space. Avoid further advancement to prevent piercing the anterior sacral wall and puncturing the dura.
