Contrast-enhanced 2D echocardiogram, zoom apical 4-chamber orientation, focused on the right ventricular (RV) apex. The center dark line represents the ventricular septum (myocardium). The unenhanced, well-circumscribed, 2.0 cm × 1.5 cm filling defect in the RV apex represents a large RV thrombus (lower arrow). On this video, it is noted to be highly mobile and leaves the imaging plane entirely for much of the cardiac cycle. A smaller 0.3 cm × 0.3 cm adjacent filling defect is seen near this large clot and likely represents a second smaller, highly mobile, thrombus. A nonmobile dark region is seen near the RV apex and is consistent with a muscle bundle or trabeculation (upper arrow). The dark, poorly circumscribed, amorphous region toward the base of the RV cavity represents attenuation artifact.
