This video clip showing two-dimensional transesophageal echocardiography (intraoperative). Partial atrioventricular septal defect (ostium primum atrial septal defect). 4-chamber view. The arrowhead points to a large ostium primum defect and the arrow to shunt flow from left to right atrium through the defect. A represents an aneurysm of the ventricular septum (actually a portion of the septal leaflet of the tricuspid valve) which is seen closing the ventricular component of the defect. The heart is practically at stand still during surgery. Air bubbles are noted in the cardiac chambers. P represents the patch used to close the defect and arrow points to the surgeon’s finger as he is checking the patch. After bypass, the patch (P) appears intact. (LA: Left atrium; LV: Left ventricle; RA: Right atrium; RV: Right atrium).
