This video clip transesophageal echocardiographic demonstrates the aortic valve at 105° profiling the large shaggy vegetation attached to right coronary cusp of aortic valve. The large vegetation is hitting the anterior mitral leaflet. Vegetation-satellite lesion is also visualized over anterior mitral leaflet. Color Doppler interrogation with color comparison shows severe mitral regurgitation across the perforation of anterior mitral leaflet and away from the central orifice of the valve.
