This video demonstrate phaco on hard cataracts. Dealing with hard nuclei is different: The goal is to apply, which is best done in burst mode or with very short periods of continuous or pulse mode. To avoid damage to the posterior capsule and the endothelium of the cornea the phaco tip should work mostly in the centre as well as in a safe distance from the posterior capsule and the cornea. For dense and hard nuclei, it is advisable to stain the capsule with Trypan Blue, prepare a larger CCC and try to avoid stress on the zonules. Breaking and emulsifying the nucleus is demonstrated in a systematic way. The smaller the pieces, the faster can they be emulsified. A common problem with hard, brown nuclei is that the central lower portion of the nucleus cannot be easily broken off and separated. Special care needs to be taken not to damage the posterior capsule. If a nucleus is too dense and hard and if a lot of phaco energy is required, then phaco is not advisable.
