This video demonstrates the authors’ technique to perform a tympanomastoidectomy for attic cholesteatoma. Although various techniques have been described for the management of attic cholesteatoma, each surgery needs to be custom-made for a particular patient. In the authors’ experience, an atticotomy, followed by an attico-antrostomy, with the mastoidectomy limited to the extent and location of the cholesteatoma has proved to be the least invasive option, provides a direct approach to the attic, usually results in small cavities with satisfactory long-term outcomes, usually without the need for a second-look surgery.
