After the conjunctiva has been opened at the limbus at 12 ‘o’ clock a 5 × 5 mm sclera flap is dissected till clear cornea. Now a 4 × 4 mm sclera flap is dissected till the channel of Schlemm. See the blue choroid thinning through the thin sclera. A keratocentesis is made to lower the IOP. Minimizing the risk of iris prolapsing if you should get a puncture of the corneal window.
