An elderly gentleman with severe kyphosis due to ankylosing spondylitis presented for cataract surgery. He could not wear eve glasses because of this face down posture, so a multifocal lens was implanted in each eye. Biaxial microincision surgery was advantages due to the ability to locate the incisions in the most ergonomic position for the surgeons, and the ability to switch instruments between the hands.
