Cases of posterior polar cataract are notorious for posterior capsular rupture. So Manual SICS in these cases is incorporated with hydro- delineation, and the minified nucleus is flipped upside down to bring it out into the anterior chamber. Hydrodissection is avoided. Nucleus and subsequently the epinucleus is visco expressed. The subincisional cortex is removed with a J-shaped cannula. A PC IOL is implanted in the bag.
