It is a great honor and pleasure for me to be invited to write the foreword for book ‘Retina: Medical and Surgical Management’ by Professor Atul Kumar. Professor Kumar is currently the Chief and Professor of Ophthalmology at Dr Rajendra Prasad Centre for Ophthalmic Sciences (RPC-AIIMS), the national apex ophthalmic center at All India Institute of Medical Sciences in New Delhi, India. He is a renowned retina specialist, particularly in vitreoretinal surgery for various diseases such as macular holes, retinal detachment, myopic traction maculopathy and so forth. In 2007, he was awarded the Padma Shri, the fourth highest civilian award in India for his contribution to the field of medicine. In 2016, he was appointed Honorary Advisor Ophthalmology to the Government of India. To date, he has published more than 260 articles, 16 book chapters and 5 books. Fortunately, I had an opportunity to become acquainted with him recently, and I soon realized how sincerely dedicated he is to ophthalmology practice, research and education. I believe that this book has been accomplished through his extraordinary passion for education, particularly of young physicians who wish to acquire the cutting-edge knowledge of the retina. Needless to say, this book also represents the efforts of many distinguished contributing authors.
After reading through the manuscript, I am convinced that this is the ideal textbook for ophthalmology residents, retina fellows and even for senior retina specialists, who want to update their knowledge of retinal diseases and treatment within a relatively short time. The content is condensed, yet core information is never omitted.
In , essential information related to the basics of science and clinical knowledge, such as the imaging findings and electrophysiology of the retina are described. From to , the pathophysiology and medical and surgical treatments of various retinal disorders are discussed in detail. Readers will appreciate the presence of many high-quality images and illustrations throughout this book, which will facilitate greater reader comprehension. This is certainly a ‘must-have’ textbook for clinicians to improve their understanding of retinal diseases.

Preface
It gives me great pleasure to bring out the second updated edition of the Retina: Medical and Surgical Management. It encompasses the recent updates and latest advances in diagnostics, clinical management guidelines, instrumentation and vitreoretinal surgeries and will provide the readers a holistic view about the retinal subspecialty which will prove to be extremely useful to the readers.
This has been possible after months of data compilation, image acquisition and revision of text with extensive inputs from senior residents and junior residents working at RP Centre, AIIMS, New Delhi, India. It was in 2016 that I conceived the idea of a comprehensive textbook on retina for residents and vitreoretinal fellows, and also a reference book to flip through, lying on the desk of the practicing ophthalmologists and so decided to put down my thoughts and ideas of all what I see and do daily since the last 30 years into a book form.
I have always been passionate about retina as a sub-specialty since my residency days at RP Centre and was amazed about this field of vitreoretinal diseases where rapid changes in technology meets newer techniques and continues to evolve rapidly expanding itself into a complex practice of ophthalmic medicine and surgery that utilizes a wide range of diagnostic and therapeutic modalities to treat ocular diseases. From case reports to phase 3 multicenter clinical trials, published data continues to accumulate in exponential fashion. As a result it is increasingly difficult for the clinician dealing with vitreoretinal disease, both general and specialist alike to apply the amassed information to patient care.
This textbook is primarily intended to provide the ophthalmology residents and practicing comprehensive ophthalmologists with an up-to-date, clinically oriented source that covers the full spectrum of medical and surgical vitreoretinal disease. Subspecialists in the field should also find it useful as a review and selected reference.
Although, this text is meant to be comprehensive, we have tried to highlight the essential, clinically important aspects of vitreoretinal medical and surgical practice. Chapter dealing with disease state emphasize clinical feature, diagnosis, and management. Entire chapters are devoted to the most commonly encountered problems. New sections about emerging role of 3D ‘heads up’ vitreoretinal surgery and microscope integrated optical coherence tomography in retinal surgeries have been added.
The text is divided into 9 sections, each section having many chapters in a systematic fashion devoted to the various retinal diseases and their signs, symptoms and management. The latest imaging platforms with illustrations and surgical techniques have been described in this textbook. Assembling a textbook that attempts to have the chapters both current and detailed in content along with uniform layout requires a special effort from all the contributors to adhere to strict guidelines and I am grateful to all those invited to participate in the writing of this textbook for their applaudable efforts.
About the Book: An Introduction
This book is intended to serve as an in-depth textbook and guide for understanding the subject of retina and vitreous and the diseases related to it with well-illustrated colored images and all the recent updates. It is broadly divided into 9 sections with relevant chapters in each section. Section 1 is dedicated to basic sciences and diagnostics and covers useful anatomy and physiology of the eye, the all important retinal imaging with the latest imaging platforms and crisp pictures of retinal diseases imaged with the latest high technology hardware. Ocular electrophysiology and ophthalmic USG which provides information of retinochoroidal disorders completes this section.
is devoted to retinal degenerations and fundal dystrophies. This includes pathological myopia and its lesions with pictorial representation, retinitis pigmentosa and related syndromes, hereditary retinal and choroidal vitreoretinopathy and dystrophies.
is totally devoted to macular diseases including central serous chorioretinopathy (CSC), age-related macular degeneration (AMD), and etiologies leading to secondary choroidal neovascular membrane (CNVM). Vitreomacular interface diseases and vitreomacular traction (VMT) are explained next, closely followed by a chapter each devoted to epiretinal membranes (ERMs), and macular holes each.
Myopic traction maculopathy is now imaged more commonly in myopic eyes especially with the swept-source optical coherence tomography (SS-OCT) device and is next described, followed by optic disc pit maculopathy and its management with internal limiting membrane (ILM) stuffing. Submacular bleeding is now seen often especially in polypoidal choroidal vasculopathy eyes and retinal arterial macroaneurysm (RAM) eyes requiring an effective management with submacular tissue plasminogen activator (tPA) injection and is described in detail. Low vision aids still plays an important role in eyes with end-stage AMD lesions and scar formation and a detailed explanation of the available aids and their usage provides a place in this book.
The next section is devoted to vascular occlusions including ocular ischemic syndrome (OIS). The all important diabetic retinopathy is explained in great detail including its epidemiology, pathogenesis, classification and trials till date. Diabetic macular edema (DME) being the prime cause for visual disturbance is discussed with its management in a separate chapter. Proliferative diabetic retinopathy (PDR) and its laser, anti-VEGF and surgical treatment comes next and then macular telangiectasia followed by miscellaneous vascular retinopathies in one chapter.

is devoted to uveal diseases including both infectious and noninfectious uveitis with illustrations to support the text.
details with surgical retina including management of retinal detachment (RD), retinal lasers, scleral buckling procedures including pneumatic retinopexy, vitreous substitutes, techniques for pars plana vitrectomy, giant retinal tear (GRT) management, vitrectomy for diabetic tractional retinal detachment (TRD), dislocated nucleus and intraocular lens (IOL) followed by complications of vitreoretinal (VR) surgery including endophthalmitis. This section ends with newer innovations and techniques in vitreoretina.
The second last section covers the useful topic of ocular trauma as and comprehensively covers classification, globe injury, disc avulsion, retained intraocular foreign body (RIOFB), post-traumatic endophthalmitis and shaken baby syndrome. The last section of retinal diseases covers the newly described entities of cancer-associated and autoimmune retinopathies, toxic and photic retinopathies and lastly phakomatosis.
This book will provide the specific academic requirements for postgraduate students as well as ophthalmic fellows pursuing retina. It shall also be useful for all the ophthalmic practitioners and researchers as it contains extensive and easily accessible material with special focus on medical and surgical management of various diseases in retina and vitreous from basics to the current concept in a comprehensive and easy-to-understand manner. I hope the book proves useful to provide a better understanding of various vitreoretinal conditions and its medical and surgical management.
