Neglected injuries of ankle and foot include soft tissue and bony injuries. Ankle sprains are among the commonest injuries sustained during athletic activities. The rationale of treatment including nonoperative, operative treatment and expected outcome is described. Neglected ankle fractures produce degeneration of articular cartilage with poor outcome following surgeries. Surgeries may vary from ankle arthrodesis to supramalleolar osteotomy to internal fixation of various malunited/uniting fracture fragments. Neglected lisfranc injuries, talar fractures, calcaneal fractures produce considerable disability on walking over all type of surfaces. The treatment may be individualized based on clinical problem. It may vary from nonoperative treatment with soft sole shoes to fusions.
