This chapter reviews the need, clinical effectiveness, and implications of oocyte donation and sperm donation. Indications for oocyte donation include premature ovarian insufficiency, Turner syndrome, natural menopause, repeated implantation failure due to poor quality oocytes, and genetic disorders. Duties, responsibilities and regulations pertaining to an assisted reproductive technology (ART) clinic are also listed in this chapter. Thorough evaluation of the recipient should be performed to rule out any abnormalities which can affect pregnancy outcome. Endocrinological investigations that affect the fertility potential and subsequent pregnancy prognosis like thyroid function test, serum prolactin, fasting glucose etc. should be evaluated. Screening of gamete donors has to be done by an independently registered ART bank. Oocyte donors should be between 21 years and 35 years of age with at least one living child of minimum 3 years age. Counseling of the donor should include details of the procedure, need for regular visits, and possible complications due to ovarian stimulation like nausea, headaches, abdominal bloating, and rarely ovarian hyperstimulation.
