The incidence of posterior migration of nuclear or cortical matter during cataract surgery is around 0.1–0.3%, the odds being almost similar with phacoemulsification, small incision cataract surgery, and extracapsular cataract extraction. Traumatic dislocation usually follows a closed globe injury, though post open globe injury has also been described. Tell-tale signs are its association with other signs of blunt injury such as traumatic mydriasis with sphincter tears, vitreous hemorrhage, commotio retinae, retinal dialysis, angle recession, and choroidal ruptures. Displacement of intraocular lens (IOL) from its normal location (in the capsular bag) may occur, varying from decentration to dislocation into the vitreous cavity. Intraoperative dislocation occurs if IOL is inserted in the presence of large posterior capsular defect or IOL haptic gets misplaced.
