Abstract

Hypertension is one of the major causes of morbidity and mortality across the world. It affects directly or indirectly, almost all the organs in the body, especially heart, kidneys, brain, and eye as target organs. Retinopathy is the most common ocular manifestation of hypertension. While classifying hypertensive retinopathy, it is important to consider the superimposed changes caused by arteriosclerosis which may be accelerated by hypertension. Choroidal ischemia acts as an underlying mechanism for hypertensive choroidopathy, which affects retinal pigment epithelium and the retina. As compared to hypertensive retinopathy, hypertensive choroidopathy is a less recognized entity. Pathophysiology of choroidopathy is similar to hypertensive retinopathy and involves fibrinoid necrosis of choriocapillaris due to elevated blood pressure leading to patchy choroidal infarcts and other signs of hypertensive choroidopathy like Elschnig spots and Siegrist streaks.
