Abstract

This chapter describes the causes for diagnostic errors in health system and electronic health records to improve the diagnostic process. Ensuring reliable and timely diagnosis represents a major challenge. Health information technology (HIT) tools and electronic health record systems have the potential to enable physicians to overcome-or at least minimize these human limitations. This chapter covers the diagnostic errors, phases of diagnostic errors, and prevention of diagnostic errors. Diagnostic error may be defined as a mistake in the diagnostic process leading to a misdiagnosis, a missed diagnosis, or a delayed diagnosis. The diagnostic testing process divided into three phases following as preanalytic phase, analytic phase, and postanalytic phase, which are briefly discussed.
