Abstract

The objective of this chapter is to introduce insulin therapy. The goal of treatment in type 1 diabetes mellitus (T1DM) is to provide insulin in as physiological manner as possible. To mimic physiologic insulin secretion, both long- and short-acting insulins are used. Long-acting insulin suppresses glucose output from the liver overnight and provides basal insulin between meals; bolus doses of short-acting insulin modulate glucose excursions associated with carbohydrate consumption. Conventional insulins and modern insulins are two types of insulins, which are briefly discussed in this chapter. The choice of insulin regimen will depend on many factors including: age, duration of diabetes, lifestyle, targets of metabolic control, and particularly individual patient or family preferences. Storing and mixing of insulin, insulin injection technique, insulin absorption, self-monitoring of blood glucose, future insulins and delivery systems are some other important topics, which are also provided in this chapter.
