This chapter discusses about the sample recipient evaluation form which constitutes a possible way to address the intake information, and should be modified according to the specific needs of each program. Recipient evaluation form include information about the existence of the patient, physician information, cause of kidney failure, location of encounter, cardiac history, allergies if any, family history, occupation, family history of kidney disease, habits, and physical examination information. Based on the filled information in the form, the recipient should acknowledge that kidney transplantation should be considered as a treatment option for the concerned patient’s end-stage renal disease, and should agree to proceed with the protocol evaluation for kidney transplantation. Patient are made aware that he or she will need to take immunosuppression medications in order to preserve the kidney for as long as the organ is functioning in order to prevent rejection.
