Abstract

The dacryostenosis corresponds to theobstruction of the lacrimal pathway, being the congenital form, the most frequent presentation of it. Approximately, 90% of the patients with this pathology presets spontaneous resolution at one year of age. The formation of the lacrimal pathway ends with the canalization of the duct. Failure in this process pleads to obstruction anywhere within the pathway; at the lacrimal punctum because of atresia, in the lacrimal sac because of obstruction in the junction of the sac with the nasolacrimal duct, or even in the duct at the exit in the nasal cavity. The obstructions are very frequent at the level of the nasal mucous. The main clinical manifestation is the epiphora and happens when the lid is unable to contain the excess. There can be infections due to repetitive secretions and also alterations in the skin. Laser approach and radiofrequency is gaining popularity for investigation. The symptoms might be present since the newborn, but the epiphora increases at the time that the tear production also increases; frequently the symptoms are only in one eye. The compression to the lacrimal sac can produce reflux of tear, mucous secretion or even purulent secretion. The medical treatment is conservative and has the objective to keep hygienic measures to maintain the eye uninfected. It is also mandatory to avoid the infection of the lacrimal sac, so a soft massage is done. Surgery through lacrimal probe and intubation through lacrimal pathway is done. Dacryocystorhinostomyand Laser Assisted Dacryocystorhinostomy can also do. The majority of the congenital lacrimal obstructions are solved in a spontaneous mode in the early months of life.
