Abstract

Management of the posterior capsule greatly influences the outcome of pediatric cataract surgery. Posterior capsule opacification (PCO), which is often referred to as an “after-cataract” or a “secondary cataract” is a common complication of pediatric cataract surgery. PCO development is age dependent, with a low incidence in older children but higher rates in younger children. To prevent PCO, posterior capsulectomy and vitrectomy are essential surgical steps. The posterior capsule can be opened using a manual technique (capsulorhexis) or it can be cut with a vitrector or any other cutting devices (vitrectorhexis), venturi pump is also recommended for cutting. Posterior vitrectorhexis can be performed via a limbal or a pars plicata approach. Treatment of the posterior capsule determines where and how safely the intraocular lens can be fixed. Selection among the various technical alternatives is probably best left to the individual surgeon’s based on the individual case, available facilities, individual circumstances, and personal experience.
