Abstract

With an incidence of 0.1%, post surgical endophthalmitis can present after cataract surgery. Gram-positive organisms are responsible for 95% cases. Presentation is with sudden onset of rapidly worsening pain along with a significant decrease of vision. Other features include excessive tearing, photophobia and blepharospasm. Signs include circumcorneal congestion, hazy cornea, ring abscess, hypopyon, yellowish vitreous exudation, etc. ultrasonography, aqueous and vitreous tap, vitreous biopsy all help to establish the diagnosis. Management options include antimicrobial (intravitreal, topical and systemic therapy), anti-inflammatory therapy (intravitreal, topical and systemic corticosteroids) supportive therapy and vitrectomy.
