Abstract

Antepartum hemorrhage is a very important condition because it entails high maternal and fetal morbidity and mortality. Initial resuscitation of the mother is important. Antepartum hemorrhage is an age old obstetric problem with high maternal mortality if not managed well. Perinatal mortality is also concomitantly high in untrained hands. Proper diagnosis is crucial as each form of antepartum hemorrhage requires its specific management. The involvement of a multidisciplinary team in a tertiary hospital that includes a senior consultant obstetrician, a trained anesthetist, a neonatologist and a hematologist is important. Prematurity is an issue, but with proper neonatal support, prognosis is better than before. The advent of high resolution grey scale ultrasound has revolutionized the field of diagnosis in the arena of antepartum hemorrhage. Antepartum hemorrhage can be due to placenta previa, abruptio placentae, local causes or indeterminate antepartum hemorrhage. The patient who is stable and has no further loss requires conservative management after locating the placenta, replace blood if hemoglobin is less and active measures like delivery is indicated if there is severe hemorrhage.
