Abstract

In this chapter, we report our experience with the Boston keratoprosthesis and the outcome after implantation of this device at the Cornea service of the Massachusetts Eye and Ear infirmary. For a number of years, we have used a PMMA keratoprosthesis of double-plated collar button design. Such a general configuration has been suggested in the past, and has been modified by us as described in the following section. Design and material are undoubtedly important, but it should be emphasized that the health of the surrounding tissue is vitally important as well. We do not have sufficient experience with other devices to do justice to them or to the experience of other surgeons, nor do we claim superiority of our prosthesis or our technique over programs elsewhere. This chapter describes the Boston keratoprosthesis devices, surgical techniques and follow-up routines with which we have become proficient. Our surgical technique and postoperative regimens, the only approach we have personal experience with, will be described in some detail. This technique has been developed over a number of years, with modifications and adjustments to improve the postoperative outcome.
