Abstract

This chapter discusses partial laryngectomy for advanced laryngeal and oropharyngeal tumors, where supracricoid partial laryngectomy with cricohyoidopexy (SCPL) is based upon the preservation of at least one functional and intact cricoarytenoid unit. Organpreservation surgery (OPS) consists of a full spectrum of surgeries, and any given laryngeal cancer may have more than one appropriate option for treatment. The glottis and subglottis arise from the tracheobranchial anlagen, while the subglottis is also a midline structure and the glottis is formed from two lateral structures, and thus tends to drain ipsilaterally into its lymphatics. During operative laryngoscopy, the tongue base and vallecula must be palpated to determine if tumor has spread to these sites. The thyroid cartilage is retracted to one side, and the contralateral inferior pharyngeal constrictor muscle is dissected from the posterior and superior borders of the thyroid cartilage.
